The client
The client is a Virginia-based network of home-visit medical providers offering primary care services to patients at home and in assisted living facilities. Their model brings medical attention directly to underserved and aging populations, prioritizing accessibility and quality of care.
Client achievements:
Client’s research has been published in over 100 peer-reviewed medical journals, reinforcing their role as thought leaders in home-based primary care.
Client’s innovative model has been featured by major outlets including Modern Healthcare, Reuters, NBC News, and ABC News, helping bring national attention to home-visit care.
The client secured NIH funding to lead a nationwide study focused on identifying undiagnosed Alzheimer’s disease and related dementias — a major step in advancing early detection efforts.
The challenge
The client’s needed a digital solution that would help patients easily find nearby, high-quality home-visit providers. The product had to be intuitive enough for elderly users and capable of handling large datasets from Medicare claims. Key requirements included:
Location-based provider discovery
Visualization of provider quality ratings and visit statistics
Geographic search functionality (ZIP code, city, proximity)
Simple, accessible UI
Support for regular data imports and updates
What was done
We delivered a lightweight but powerful MVP in just one month. The platform integrates Medicare claims data, provides real-time search across 100K+ practices and 200K+ providers, and is fully optimized for mobile use. A smooth knowledge transfer followed, enabling client’s team to take over with no disruption.
Implemented features:
Smart location-based search
Users can instantly view the five closest providers, regardless of city boundaries. The search system includes ZIP code and city autocomplete, detects user location on the first visit, and filters results within a 15-mile radius. Built with MongoDB’s geospatial indexing, it uses spherical distance calculations for precision.
Provider display
Active providers (those with recent visits) are prioritized in results. Listings show 5-star quality ratings, exact city matches, nearby alternatives, and distances in miles — all grouped in collapsible sections by city for easier browsing.
Practice information integration
The platform supports seamless imports of Medicare claims data via Excel files, with automatic geocoding of practice locations and monthly data updates built in.
Provider ratings system
Providers are rated using a 5-star visual system based on care quality, visit statistics, and performance indicators. This helps users make more informed choices at a glance.
Practice-level detail view
Comprehensive data is provided for each practice, including provider rosters, visit counts split by home or assisted living, and patient demographic insights.
User interface features
The front-end is fully mobile-responsive with a card-style result layout. The interface auto-expands the first result for faster browsing and includes scroll-to-top functionality to improve navigation on longer search pages.
Business value
The platform delivers practical benefits for both patients and providers by removing barriers to access and improving decision-making through data-driven design. Its focus on location accuracy, visual clarity, and responsive performance helps users get faster answers — and better care.
Reduces travel time for home visits
Visualization of provider quality ratings and visit statistics
Improves service coverage visualization
Facilitates better provider–patient matching
Immediate results without city boundary restrictions
Clear distance indicators
Quality metrics at a glance
Mobile-responsive design
